Position Summary
This is a full-time position responsible for leading and coordinating housing case management services under the Housing Opportunities for Persons with AIDS (HOPWA) and HOME-ARP programs at Back Bay Mission.
This federally funded program is designed to deliver comprehensive case management services that promote housing stability and long-term self-sufficiency for program participants.
Education
Bachelor’s or master’s degree in social work preferred. Relevant education and/or comparable experience will be considered.
Experience
Two (2) or more years of experience providing case management within a public or nonprofit social service agency preferred. Experience working with individuals experiencing homelessness or housing instability is highly desirable.

a) A genuine concern for individuals and families at risk of homelessness or currently unsheltered.
b) Demonstrated empathy and understanding of the challenges faced by individuals experiencing economic crisis.
c) Ability to effectively manage, coordinate, and deliver housing-related services to promote stability and self-sufficiency.
d) Strong documentation skills with the ability to complete client records within 24–48 hours of service delivery.
e) Ability to work collaboratively with coworkers, supervisors, and community partners.
f) Strong communication skills with the ability to build and maintain community partnerships.
g) Flexibility, patience, and adaptability in a fast-paced environment.
h) Excellent organizational and multi-tasking abilities.
Tasks and Responsibilities
a) Maintain active working relationships with community social service providers and partners.
b) Attend monthly Continuum of Care (CoC) meetings with the Open Doors Homeless Coalition.
c) Conduct comprehensive assessments of individuals and families experiencing or at risk of homelessness.
d) Assist individuals not eligible for services by connecting them to appropriate alternative resources.
e) Work collaboratively and closely with the Micah Day Center staff and Health and Wellness Navigator.
f) Collaborate with Back Bay Mission case management staff, including ESG and HUD Home At Last Case Managers.
g) Develop and maintain a comprehensive understanding of HMIS and all program reporting requirements.
i) Participate in data collection and reporting for Board of Directors reports, including monthly, quarterly, and annual reports in collaboration with the Lead Housing Case Manager.
j) Maintain accurate and timely client records and program data in HMIS in accordance with HOPWA and HOME-ARP requirements.
k) Determine client eligibility and enroll participants in HOPWA and HOME-ARP programs in compliance with grant guidelines.
l) Develop and implement individualized service plans focused on achieving housing stability.
m) Coordinate and deliver case management services including service planning, referrals, monitoring, and follow-up.
n) Generate required program reports (monthly, quarterly, annual, and as needed for compliance.
o) Participate in Outreach to the unsheltered with other Back Bay Mission Staff and CoC members.
p) Be aligned with the Back Bay Mission Housing Case Management Standards,.
q) Engage with volunteer mission groups and campers as needed to share program insights and impact.
r) Participate in additional Back Bay Mission activities as assigned by the Lead Housing Case Manager or Executive Director.Back Bay Mission
